Dr. Niloofar Zarkesh
Dr. Niloofar Zarkesh is a board-certified periodontist with more than two decades of experience in periodontal and dental implant care. She is the founder of Perio4U, a San Jose-based periodontics specialty practice known for providing compassionate, patient-focused care using the most advanced techniques in periodontology and dental implants.
As both a clinician and practice leader, Dr. Zarkesh blends technical excellence with empathy. She is deeply committed to creating a warm, respectful, and welcoming environment for her patients — many of whom have been under her care for years. Her calm demeanor, attention to detail, and passion for preserving natural teeth have earned her the trust of patients and referring dentists alike.

Dr. Zarkesh was born and raised in Iran and moved to the U.S. in the early 1990s to continue her dental education and pursue her dream of becoming a specialist. She lives in the Bay Area with her husband and their two children, a son and a daughter, who keep her life filled with joy, learning, and laughter.
When she's not in the office, Dr. Zarkesh enjoys Photography, Snow skiing – and Volunteering for the dental community.

Dr. Adrienne Gunstream is a board-certified periodontist who specializes in the diagnosis, prevention, and treatment of periodontal diseases, as well as the replacement of missing teeth with dental implants. She holds Diplomate status from the American Board of Periodontology, a distinguished designation reflecting clinical excellence in the field. Dr. Gunstream practices part-time at Perio4U Periodontics and Dental Implant Center in San Jose, California.

By utilizing the latest technologies in dental imaging, digital treatment planning, regeneration, and minimally invasive surgical techniques, I am confident in the effectiveness and predictability of the treatments I provide.
I particularly enjoy esthetic periodontal procedures, including gingival grafting and implant placement. At the same time, diagnosing and treating periodontal disease to help patients preserve their natural teeth is one of the most rewarding aspects of my work.”
Vida Jabbari, Office Manager
Vida Jabbari is our Office Manager, bringing a unique blend of clinical and administrative experience to our practice. She began her dental career as a Registered Dental Assistant (RDA) after earning her Dental Assisting certificate from San Jose City College. She also holds certifications in x-ray, coronal polishing, radiology safety, and infection control. Vida is CPR-certified and fully compliant with HIPAA regulations.
As Office Manager, Vida oversees the day-to-day operations of our practice, ensuring an efficient and welcoming environment for both patients and staff. She is a trusted resource for financial coordination—helping patients understand and maximize their insurance benefits, explore payment options, and access the care they need. With meticulous attention to detail, she submits insurance claims with the necessary documentation to help ensure timely and complete reimbursement. As a strong advocate for our patients, she works diligently to help them make the most of their coverage.
With exceptional organizational skills, comprehensive dental knowledge, and a commitment to outstanding service, Vida plays an essential role in our practice. We are proud to have her leading our team and supporting our patients every step of the way.
Han Duong, RDH
Han Duong, RDH is our dedicated Registered Dental Hygienist, licensed by the State of California and a graduate of the Dental Hygiene Program at Chabot College. Her clinical scope includes dental prophylaxis (cleaning), scaling and root planing (deep cleaning), periodontal maintenance, fluoride treatments, desensitizing agents, and the administration of both topical and local anesthetics when needed.
Education is a key part of her approach. She takes the time to help patients understand how to maintain healthy teeth and gums at home, empowering them with the knowledge and tools they need to succeed.
She is CPR-certified and fully compliant with HIPAA regulations. Known for her exceptional clinical skills and attention to detail, she delivers personalized, high-quality care focused on both prevention and long-term oral health.

Realyn Paas, RDA
Realyn Paas, RDA is a Registered Dental Assistant with certifications in x-ray, coronal polishing, radiology safety, and infection control. She graduated from the Dental Assisting Program at Everest College in 2012. She is CPR-certified, fully adheres to HIPAA regulations, and is fluent in both English and Tagalog.
Currently serving as our Front Office Assistant, she is often the first point of contact for patients—answering calls, scheduling appointments, verifying insurance benefits, and coordinating care with other dental and medical offices. She also manages our hygiene schedule to help ensure efficient patient flow and timely care.
In addition to her front office responsibilities, she is trained and experienced in assisting with a variety of periodontal surgical and non-surgical procedures. Her calm and caring demeanor helps ensure that patients feel comfortable and well-supported throughout their visits.

Lara Alcantara, DA
Lara Alcantara, DA is a skilled Dental Assistant with certifications in x-ray, coronal polishing, radiology safety, and infection control. A graduate of the San Jose City College Dental Assisting Program, she is CPR-certified and fully compliant with HIPAA regulations.
She assists the doctor during a wide range of periodontal surgical and non-surgical procedures, ensuring each patient feels comfortable, supported, and well cared for throughout their visit.

Mahtab Baghai, RDA is a Registered Dental Assistant with certifications in x-ray, coronal polishing, radiology safety, and infection control. In addition to her clinical training, she earned a Bachelor’s degree in Biology from the University of California in 2006. Fariba Yousefi, is a dedicated and compassionate Dental Assistant who has been an integral part of our practice since 2004. She earned her Dental Assisting certificate and x-ray license from San Jose City College and holds an Associate of Arts (AA) degree. She is CPR-certified and fully adheres to HIPAA regulations. Fluent in both English and Farsi, she helps ensure clear communication and a welcoming environment for all patients.
In addition to her clinical expertise, Fariba coordinates continuing education programs for our referring dental offices and plays a key role in developing our marketing and patient education initiatives. Over the years, she has built strong relationships with many of our referring doctors and their staff, serving as a trusted liaison and the familiar face of our practice.
Fariba has contributed in many capacities throughout her time with us, always demonstrating professionalism, loyalty, and a deep commitment to our patients and colleagues. We are proud to have her on our team and grateful for her longstanding dedication to our practice.
